Blue Choice Preferred PPOSM

Best Fit: 2-50 Employees

Blue Choice Preferred PPO is a comprehensive PPO network of doctors and hospitals often with a higher monthly premium but lower out-of-pocket costs.

BluePrint PPOSM

Best Fit: All Group Sizes

Access to more than 23,000 doctors and over 200 hospitals. Members can choose any doctor or hospital. Benefits include hospital/surgical care, maternity care and diagnostics testing.

Blue Choice Select PPOSM

Best Fit: 51-150 and 151+ Employees

Includes all the features of a PPO plan. The difference that sets Blue Choice Select PPO apart is that it uses the Blue Choice PPOSM network which is a select, more affordable network than the larger PPO network.

The HMO network provides basic care and prescription medication coverage while coordinating with care specialists for any necessary referrals. Employees have a choice of Primary Care Providers from a large network of contracting HMO physicians and medical groups.

Blue Advantage HMOSM

Best Fit: 51-150 and 151+ Employees

Provides basic medical care and medication. Coordinates care and treatment with specialists and provides appropriate referrals. Flexible plan design options with deductible/coinsurance and copay features

Blue Precision HMOSM

Best Fit: 2-50 Employees

Members must live or work in the Chicago, Peoria, and partial Rockford rating areas. They must choose a contracting medical group who provides coordinated care and referrals for specialist care.

HMO of Illinois

Best Fit: 151+ Employees

One of the largest provider networks in the state. Members choose a medical group to provide coordinated care. Flexible plan design with selection of deductible, coinsurnace and copay options.
